About Crossroads Hospice

Located at 101 Noons Creek Drive in beautiful Port Moody, our 10-bed Hospice is run in partnership with Fraser Health. Our Hospice allows individuals to live their final days with peace and comfort in a home-like setting where family, friends and pets are welcome.

Our Facility

Each of our ten private bedrooms is fully furnished and provides the option for family members to stay overnight. Only steps from each bedroom is our Great Room, a living and dining area for loved ones to gather together to enjoy a meal, spend time with a puzzle, or get crafty. While in the Great Room, the air will be filled with the aroma of home-cooked meals being prepared for patients, and baked goods for families and friends. Our Hospice also has two beautiful rooftop gardens, maintained with love and care by our amazing Volunteers.

Our Care Team

Several people make up the Care Team at Crossroads Hospice. The Care Team is comprised of Fraser Health staff, Crossroads Hospice Society staff, and Hospice Society volunteers. Fraser Health is responsible for the medical and nursing care of all patients and provides a team of doctors, nurses, and a pharmacist. Fraser Health also provides a spiritual care coordinator and social worker to address psychosocial and spiritual needs.

Crossroads Hospice Society is responsible for facility management and provides the non-medical support and services including kitchen and housekeeping operations. The Society also provides emotional and psychosocial support for patients and their families, in addition to complimentary programs.
